New FMLA Forms Available

In recent days, the U.S. Department of Labor (DOL) published new forms recommended for administering the Family and Medical Leave Act (FMLA).  Since March 1, 2015, employers have been using forms that expired on February 28.  The new forms can be found on

The new forms are the same as the old forms, except for the addition of a reference to the Genetic Information Nondiscrimination Act (GINA) in the instructions to health care providers.  The new instructions include information for health care providers that prohibit them from sharing information derived from genetic tests or diseases or disorders associated with an employee鈥檚 family members.  The new forms are valid through May 31, 2018.
